ABSTRACT

OBJECTIVE@#To analyze clinical response of the Rituximab-based chemotherapy and prognostic features in patients with primary gastric diffuse large B-cell lymphoma (PGDLBCL).@*METHODS@#From June 2008 to December 2020, the data of 53 PGDLBCL patients were analyzed retrospectively.@*RESULTS@#The median age was 46(25-77) years old in 53 patients including 35 males and 18 females. Stomachache is the most common symptom. The diagnosis were confirmed in 47 patients by endoscopic biopsy and 6 patients by surgery. Twenty-six patients had Ⅰ/Ⅱ1 stage (Lugano staging system) disease and 27 cases had II2/IV stage disease. All patients were treated with chemotherapy, including RCHOP (25/53) and R-DA-EPOCH (28/53). Complete remission rate was 79.2%(42/53). The 3-year and 5-year overall survival (OS) rates were 77.4% and 69.8%. Univariate analysis showed that lactate dehydrogenase(LDH), Lugano stage and lesion size affected OS. Multivariate Cox regression analysis revealed that IPI score and Lugano stage were independent prognosis risk factors affecting OS. The patients in the R-DA-EPOCH group presented better survival outcomes than those in the RCHOP group with late stage (P5-year OS=0.035).@*CONCLUSION@#Rituximab in combination with chemotherapy is the backbone of therapy for PGDLBCL. IPI score and Lugano stage are independent prognosis risk factors affecting OS of PGDLBCL. R-DA-EPOCH can be superior to R-CHOP as a first-line regimen in PGDLBCL patients with late stage.

ABSTRACT

OBJECTIVE@#To investigate relationship of miRNA-132, miRNA-256, miRNA-143 and miRNA-145 level with antophagy and apoptosis of multiple mgeloma cells.@*METHODS@#Human myeloma cell line U266 and normal CD138 plasma cells were selected and used for study and detection, the 45 cases of MM were enrolled in MM group, and 40 normal persons were sellectod in control group. The expression of miRNA-132, miRNA-125b, miRNA-143 and miRNA-145 were measured by using qPCR, the expressions of autophagy-related protein (LC3-Ⅱ, LC3-Ⅰ, P62, beclin-1) and apoptosis-related molecules (cleaved-Caspase3, cleaved-Caspase7, BCL-2, BAX) were measured by using Western blot, respectively. The rate of apoptosis was measured by using flow cytometry. The correlation of miRNA expression level with clinical-related indexes including M protein, hemoglobin, β2-MG, lactate dehydrogenase, albumin, creatinine and serum calcium was analyzed.@*RESULTS@#Compared with normal plasma cells, the expression of miRNA-132 and miRNA-125b in myeloma cells increased significantly (P0.05). The expressions of miRNA-132, miRNA-125b, miRNA-143 and miRNA-145 were significantly different between DS and ISS staging group, also between the patients with abnormal and normal chromosome karyotype (P<0.05). The miRNA-125b and miRNA-143 significantly correlated with the levels of β2-MG, albumin and hemoglobin (P<0.05).@*CONCLUSION@#The expressions of miRNA-132, miRNA-125b, miRNA-143 and miRNA-145 in patients with multiple myeloma closely relate with the clinical characteristics. Both over-expression of miRNA-125b and down-expression of miRNA-143 inhibit the apoptosis of myeloma cells by up-regulation of autophagy.

ABSTRACT

This study was aimed to investigate the effect of bortezomib combined with bisphosphonates on serum levels of DKK-1 and RANKL in multiple myeloma patients, and to evaluate its role in the therapy of osteolytic lesion. Fourty-three patients with newly diagnosed and relapsed myeloma were divided into 2 groups. Twenty-three patients were treated with bortezomib combined with bisphosphonates (A group) and 20 patients were treated with bisphosphonates combined with traditional chemotherapy (B group). Serum levels of DKK-1 and RANKL were measured by ELISA before and after 4 cycles of chemotherapy. The results indicated that serum DKK-1 level significantly decreased in patients of A group (43.2 µg/L before vs 30.4 µg/L after 4 cycles of chemotherapy), and so did for serum RANKL level in A group (0.83 pmmol/L before vs 0.45 pmmol/L after 4 cycles of chemotherapy). While there was no significant differences in DKK-1 and RANKL serum level before therapy between A and B groups, but there was significant differences in DKK-1 and RANKL levels after 4 cycles of chemotherapy (P < 0.05). It is concluded that bortezomib combined with bisphosphonates obviously reduce the serum levels of DKK-1 and RANKL, thus has beneficial effect on osteolytic lesion.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the application of ultrasonography in diagnosis and treatment of the upper arm radial neuritis.</p><p><b>METHODS</b>From 2005.12 to 2011.7, 10 patients of the upper arm radial neuritis were selected and included 6 males and 4 females with an average age of 32 years old ranging from 20 to 40 years. The course of disease ranged from 4 months to 2 years. All patients feel pain on the outside of upper arm. Medical examination showed Tinel's sign masculine, and the muscle force included extension of wrist and fingers were reduced. Upper arm radial neuritis were diagnosed by clinical sign and medical examination. All patients underwent ultrasonography examination, electrophysiology examination and operation. The ultrasonography were compared to the results of electrophysiology examination,and ultrasonography were compared to intra-operative findings and pathology examination.</p><p><b>RESULTS</b>The ultrasound images showed average diameter of affected limb radial nerve were (0.29+/-0.04) cm, average area were (0.23+/-0.05) cm2, and all of these were greater than uninjured side. Ultrasonograghic findings and orientation of radial neuritis were consistent with intra-operative findings,and the results of ultrasonography were consistent with the results of pathology examination.</p><p><b>CONCLUSION</b>Ultrasonography examination provide morphological evidence for the diagnosis and treatment of the upper arm radial neuritis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the proliferation-inhibiting and multidrug-resistant reversing effect of bortezomib on human HL-60 cells, and to explore the mechanism of bortezomib-induced proliferation inhibition in human leukemia cells.</p><p><b>METHODS</b>The multidrug resistant leukemia cell lines HL-60/DNR and HL-60/VCR cells were used as models, and sensitive HL-60 cells as a control. The cytotoxicity of bortezomib on HL-60, HL-60/DNR, HL-60/VCR cells were measured by MTT method, and the non-cytotoxicity dose was determined as reversible dose. The cells were divided into 4 experimental groups: HL-60/DNR + DNR, HL-60/DNR + DNR + bortezomib, HL-60/VCR + VCR, HL-60/VCR + VCR + bortezomib. The bortezomib resistant reversal fold was calculated. The levels of XIAP, cIAP-1, and cIAP-2 mRNA and proteins expression and the activation of NF-κB of the HL-60/DNR, HL-60/VCR cells were examined by quantitative real time RT-PCR and western blot respectively after treated with gradually increasing concentrations of bortezomib (10, 40, 80 nmol/L) for 48 hours.</p><p><b>RESULTS</b>Bortezomib inhibited the cell growth of HL-60, HL-60/DNR, and HL-60/VCR in a concentration-dependent manner. The IC(50) values were (28.90 ± 3.99), (81.19 ± 9.34), and (73.48 ± 8.94) nmol/L, respectively. After treated with 10nmol/L bortezomib for 48 hours, the IC(50) value of DNR to HL-60/DNR decreased from (12.90 ± 1.75) µmol/L to (3.54 ± 0.57) µmol/L (P < 0.01), and that of VCR to HL-60/VCR from (33.25 ± 7.28) µmol/L to (9.97 ± 1.15) µmol/L (P < 0.01). The reversal fold (RF) values were 3.32 ± 0.53 and 2.64 ± 0.28, respectively. Bortezomib down-regulated the levels of XIAP, cIAP-1, and cIAP-2 mRNA and protein expression and inhibited the NF-κB activation in a concentration-dependent manner.</p><p><b>CONCLUSION</b>Bortezomib can inhibit the proliferation of HL-60 cells and reverse multidrug-resistance in the cells. The possible mechanism is associated with down-regulation of IAPs expression.</p>

ABSTRACT

Betulinic acids are lupine-type pentacyclic triterpenoid saponins commonly found in some plants of Betulaceae family, especially in the bark of betula alba (birch). The potent anti-HIV and anti-tumor activities of betulinic acids have been greatly concerned. The natural betulinic acids include betulinic acid, 23-hydroxy betulinic acid, betulin and so on. Some investigations on the structural modifications of betulinic acids were carried out, and many derivatives with excellent biological activity have been obtained nowadays. In this paper, the research advances of the structural modification of betulinic acids, as well as their anti-HIV and anti-tumor activities are reviewed.

ABSTRACT

<p><b>UNLABELLED</b>OBJECTIVE; To clone the fimA gene of Porphyromonas gingivalis (P. gingivalis) and detect its expression in Escherichia coli (E. coli).</p><p><b>METHODS</b>The fimA gene was obtained by PCR from the genome of P. gingivalis to construct a prokaryotic expression plasmid pT-BAD/fimA. pT-BAD/fimA was transformed into E. coli BL21 (DE3) competent cells and the recombination protein was characterized by means of matrix assisted laser desorption ionisation time-of-flight mass spectrometry (MALDI-TOF-MS) analysis. The bound protein was eluted with different concentrations of imidazole (250, 200, 150, 100, 50 micromol x L(-1)) respectively.</p><p><b>RESULTS</b>DNA sequencing showed that the fragment was 99.9% consistent with that of the published. After induction with L-arabinose, a new 3.8 x 10(4) protein appeared on SDS-PAGE gel. The protein was further identified by MALDI-TOF-MS. Purity of 95% of the target protein was purified by Ni-NTA Purification System after eluted with 100 micromol x L(-1) imidazole.</p><p><b>CONCLUSION</b>The fimA gene of P. gingivalis was cloned successfully and its protein was expressed correctly in E. coli. A high purity of protein FimA was obtained and it could be applied for follow-up researches.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the efficacy of continuous propofol infusion via the common carotid artery for general anesthesia.</p><p><b>METHODS</b>Forty adult patients scheduled for abdominal surgery were randomly assigned into 2 groups to receive propopol via the common carotid artery (IC group, n=20) or via the median cubital vein (IV group, n=20). Anesthesia was induced with intravenous administration of drugs and maintained with continuous propofol infusion via the common carotid artery or the median cubital vein, with the CSI stabilized at 40-/+5 till the end of the operation. During the anesthesia, intravenous injection of fentanyl (3 microg.kg(-1).h(-1)) and vecuronium (50 microg.kg(-1).h(-1)) were given intermittently to maintain the analgesia and muscular relaxation. The dose of propofol used, hemodynamics and recovery of the patients were observed.</p><p><b>RESULTS</b>The dose of propofol used during the surgery to maintain a CSI of 40-/+5 was significantly lower in group IC and than in group IV (2.57-/+0.67 vs 5.72-/+1.37 mg.kg(-1).h(-1), P<0.01). In group IC, the blood pressure was elevated in more than half of the patients and in some cases, the elevation exceeded one third of baseline value and needed intervention with hypotensive drugs. In the IV group, the patients' blood pressure remained stable and varied within the amplitude of 15% of the baseline level. Recovery of spontaneous breathing and consciousness was more quickly in group IC than in group IV (P<0.05).</p><p><b>CONCLUSION</b>Loss of consciousness and nervous reflex can be achieved with propofol infusion via the common carotid artery, which reduces propofol dose by about 50% in comparison with intravenous infusion and allows more rapid recovery of spontaneous breath and consciousness.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To optimize the synthetic procedure of S-(+)-rivastigmine hydrogentartrate which was known as an agent for the treatment of Alzheimer disease.</p><p><b>METHODS</b>S-(+)-rivastigmine hydrogentartrate was synthesized by using 1-(3-hydroxyphenyl)ethanone as the starting material via oximation, reduction and N-methylation to produce the key intermediate 3-1-dimethylaminoethylphenol, which finally reacted with N-ethyl-N-methylcarbamoyl chloride. The enantiomers were resolved with di-(+)-p-toluoyl-D-tartaric acid, and the title compound was prepared by mixing S-rivastigmine base with L-(+)-tartrate.</p><p><b>RESULTS</b>The total yield of S-(+)-rivastigmine hydrogentartrate was 4.17%.</p><p><b>CONCLUSION</b>The materials in this procedure are all commercially available. The reaction conditions are mild and total yield is high.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the inhibition of telomerase activity and cellular proliferation in tongue cancer TCCA-8113 cell lines by antisense human tankyrase-1 RNA treatment, and explore the possibility of the tankyrase-1 as a target of gene therapy for tongue cancer.</p><p><b>METHODS</b>The replication deficient retrovirus expressing tankyrase-1 antisense RNA was constructed to infect the TCCA-8113 cells. Tankyrase-1 expression was examined by RT-PCR. Telomerase activity was assayed by telomerase repeat amplification protocol (TRAP). Cell proliferation was investigated by cellular growth curve. Cellular apoptosis was detected by flow cytometry method and invert microscope.</p><p><b>RESULTS</b>Tankyrase-1 expression and telomerase activity of tongue cancer TCCA-8113 cells were significantly inhibited. There was G(1)-S phase arrest when TCCA-8113 cells were treated with antisense tankyrase-1 transduction. Cellular proliferation was arrested, and cellular apoptosis occurred after antisense tankyrase transduction.</p><p><b>CONCLUSIONS</b>The transduction of antisense tankyrase-1 by retroviral vector can significantly inhibit the tankyrase-1 expression and telomerase activity of tongue cancer TCCA-8113 cell lines, and arrest the cellular proliferation and promote cellular apoptosis. The tankyrase may be a potential target of gene therapy for tongue cancer.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effects of common carotid artery and femoral vein administration of propofol for electrocerebral silence (ES) on circulatory and respiratory function and the dosage of propofol required during ES in dogs.</p><p><b>METHODS</b>Eight dogs were anesthetized by common carotid arterial and femoral vein administration of propofol for ES for 1 h. The time of consciousness loss and recovery, dose of propofol during ES, mean arterial pressure, heart rate, respiration rate, end-tidal carbon dioxide, SpO2, cerebral state index (CSI) and anal temperature were continuously monitored. Changes in the outcome variables were analyzed at 7 time points, namely the baseline, upon loss of consciousness, at 10 s, 30 min and 1 h of CSI=0, and recovery of CSI and consciousness.</p><p><b>RESULTS</b>Carotid artery propofol administration produced ES with only half of the dose for intravenous administration. Compared with the baseline values, the mean artery pressure and respiration rate remained unchanged or decreased transiently during ES with carotid artery propofol administration. In contrast, intravenous propofol administration resulted in systemic hypotension and severe respiratory depression.</p><p><b>CONCLUSION</b>Carotid artery propofol administration produces ES with a much smaller dose than intravenous propofol administration without causing systemic hypotension or respiratory depression.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the adjustable oblique position with prone position in microendoscopic discectomy (MED) and to evaluate its advantages.</p><p><b>METHODS</b>One hundred and twenty-eight patients who underwent MED during August 2000 and August 2002 were studied. Forty-seven cases were positioned prone, 81 cases were positioned with adjustable oblique position. Operating time, blood loss and patient endurance were recorded and compared. The airway resistance of 29 lumbar disorder patients was measured when they were positioned supine, lateral and prone respectively after general anesthesia.</p><p><b>RESULTS</b>There was statistically significant difference between 2 groups in operating time, blood loss and endurance. In prone position group, when operating time lasted for more than 50 min, 75% patients felt hurt on the skin of rib arch, and 38% patients presented dizzy and nausea. And all the patients had erythema on the rib arch. In the adjustable oblique position group, 3 cases had shoulder pain after operation, and 1 case had 4 days' common poroneal nerve paralysis. The airway resistance of patients with prone position was significantly higher than those with supine and lateral positioned patients. (P < 0.05).</p><p><b>CONCLUSION</b>The adjustable oblique position in MED had the following advantages: (1) Do not limit the movement of chest and help reduce intra-abdominal pressure and epidural bleeding; (2) Give the lumbar spine the largest degree of flexion and the largest interlaminar space, which prevents injury of facet joint; (3) Increase posterior intervertebral space and make disectomy easier and exposure of nerve root and nerve root canal clearer; (4) The patients feel comfortable and can adjust the position of lumbar spine and lower extremity; (5) The monitor has the same direction with doctor, and it is easy for the doctor to operate; (6) Doctor can sit during operation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the feasibility of diagnosing parotid disease with magnetic resonance sialography (MRS) and to select the optimal scanning sequence.</p><p><b>METHODS</b>Twenty-three patients with parotid gland disease and 5 normal adults received sialography using magnetic resonance imaging technique and several sequences (including IR-FSE, FSE, SS-IR-FSE, SS-FSE) were used. After first scanning, the patients were scanned respectively 3 and 10 minutes after buccal application of vitamin C. And MR images of duct obtained. The images of parotid duct system were analysed and evaluated according to their displaying effects. Qualitative diagnosis was made based on MRI and those diagnosis were compared with pathological diagnosis after operation.</p><p><b>RESULTS</b>Images of MR sialography clearly displayed the main duct and its branches of parotid gland and the pathological changes of duct, such as narrow, expanded, stoppage. Of the scanning sequences, IR-FSE was superior to others in manifesting the parotid gland duct (P < 0.05). The performance of images after being given vitamin C did not significantly improve the displaying effect. The accurate rate of qualitative diagnosis was 95.6%.</p><p><b>CONCLUSIONS</b>MR sialography can clearly display the parotid ducts and their pathological changes. The accurate rate of qualitative diagnosis of parotid disease was higher than that X-ray sialography.</p>

ABSTRACT

Objective To observe the efficacy of prophylactic use of gramsetron(Gra) combined with dexamethasone(DEX) on preventing postoperative nausea and vomiting(PONV) after umbilical one-trocar laparoscopic appendectomy(UOTLA)in children.Methods Sixty children requring UOTLA under general anesthesia were allocated randomly into 3 groups,and each of group included 20 children.Group saline(C),group Gra 40 ?g/kg,group Gra 40 mg/kg and group DEX 0.2 mg/kg(Gra+DEX) were respectively injected intravenously after appendectomy performed outside the abdominal cavity during the operation.Results The incidence of PONV decreased significantly in group Gra and group Gra+DEX than that in group C(Pa
